Assignment 12.5: Creating Your Questionnaire Form & Sending It Out

After watching the Forward Forms video create your first questionnaire. Bryanna explains how to do this so if you’d like to revert back to the video you a free to do that. 

You are absolutely free to add your own spin to these forms. Below is an example of a questionnaire form for extension clients. If there is something you’d like to add or takeaway that is totally okay! Make it your own and do whatever you think is best for your clients.

By default with every form you create you will be required to collect the clients Full name, email addres & phone number. These are required and cannot be deleted.

EXAMPLE:

1. Full Name
2. Email Address
3. Phone Number
4. What service are you interested in? (Single Choice)
5. Have you had extensions before? (Yes or No Question)
6. What length are you wanting? (Single Choice)
7. Upload Current Hair Pictures (File Upload)
8. Upload Screenshots of Goals (File Upload)

How to Submit Your Assignment

1. Open up your text message on your phone
2. Click into the message thread with your mentor
3. Click on the image icon at the bottom right of the screen
4. Find image on your phone or take a photo by clicking the camera button
5. Click image and it will automatically upload
6. Make sure to label each image by sending a message before or after stating which assignment the image is for
